REQUESTING PROXY ACCESS
With proxy access, you can view certain medical information about your child(ren) or other adults, as permitted.
If you are a parent or legal guardian of a minor child under the age of 14 and would like to request proxy access to health information in their MyChart account, you must have your own MyChart account.
From your MyChart account, you can request full view access by choosing Proxy Request from the menu. You will need the below information about your child to complete the request:
Full name
Date of birth
Address
Name of child's primary care provider
You will be notified via MyChart if your proxy request has been accepted.
MyChart accounts for teenagers between 14 and 17 years old can only be activated by the teen themselves. Teens can choose to grant limited view access to an adult. If a parent or legal guardian already has full view access to a child's account , the parent or legal guardian’s proxy access will transition to limited view access on the teen’s 14th birthday. Granting MyChart proxy access follows New Mexico state law, giving teens between 14 and 17 years old the legal right to consent to certain types of medical treatment without the consent of a parent or legal guardian. Presbyterian respects the rights of teenage patients to keep their health information private.
Teenagers with complex medical conditions: Parents or legal guardians must contact the teen’s provider to discuss full view access to the teen’s MyChart account. Both the provider and the teen must agree to full view access before it will be given.
On the teenager’s 18th birthday, their parent or legal guardian's proxy access will end. Now considered a legal adult, the patient can then choose to re-grant limited or full view proxy access using their MyChart account by selecting Share My Record.
If you are an adult and would like to grant limited or full view proxy access to your MyChart account, select Share My Record from the menu.
Limited view proxy access to another adult’s MyChart account may also be requested by completing the Authorization to Grant MyChart Proxy Access for Adult Patient form. You can request a copy of this form at your next visit. The adult whose record is requested be made available must agree to provide access, unless a medical power of attorney or permanent legal guardianship is in place.
You will need the below information about yourself and the adult you would like proxy access for to complete the limited view access request:
Full name
Date of birth
Address
