 *Due to the Thanksgiving holiday, CNYFC will be CLOSED on Thursday, November 28th AND Friday, November 29th.
 
**There will be a Doctor ON CALL for any medical emergencies. Please call our regular phone line (315-463-1600) and speak with a Triage Nurse if you need medical assistance.
***We will resume our regular office hours on Saturday, 11/30 from 9am to 2pm, and Sunday, 12/1 from 10am to 2pm.

If you need help getting on to our Patient Portal, please go to this resource for assistance before you try to call our office for help. THANK YOU!

Our "PATIENT CHARGE FOR FORM COMPLETION POLICY" can now be found on our website under the Patient Services tab. Due to the time and resources it takes our staff to complete these individual forms for our patients, each patient will now need to pay a fee WHEN THEY DROP OFF FORMS to be completed by our staff and providers. Please go to our website for the details which identify each form and what the corresponding fee will be for each form(s).

We have added a few new "LINKS" to our website that we hope will be a good resource for our patients:
1) Learn about the recommended  vaccines for your child by clicking on the link to the CDC site. Take a quiz that will help you evaluate what vaccines your child might need to get based on their current age.

2) Learn about Health Eating recommendations from the American Heart Association, for dining at home as well as when you go out to eat at a restaurant.

3) View what a "Healthy Plate" consists of - as recommended by those at Harvard Health Publishing.

NEW SELF PAY POLICY - If you are a self pay patient, please read the new self pay policy in effect for all patients who are seen at CNY Family Care. It can be found under the Patient Services tab at the top of our  website.

PATIENT PORTAL:
If you are interested in having access to the "PATIENT PORTAL", call our office or ask at your next office visit how to become connected.

The Patient Portal allows each patient to view his/her medical history, update personal information (address, phone etc.), view recent lab results, request prescription refills and send non-emergency messages to his/her doctor's nursing team.

All you need to get connected is internet access on your home computer and the individual access code that will be assigned to you when you call our office.  Please be sure to ask your provider about this new option the next time you are here to be seen.

NEW CHILD PATIENT INTAKE FORM
All parents of children under the age of 18, WHO ARE NEW TO THE PRACTICE, need to complete this patient information form and hand it to the receptioninst when they arrive for their first appointment with the doctor.

NY STATE HEALTH PROXY FORM
This form allows you to appoint someone you trust to make health care decisions for you if you lose the ability to make decisions for yourself. Please complete and bring to CNY Family Care so it can be attached to your patient record.

RETURNING ADULT PT UPDATE FORM
ATTENTION TO ALL RETURNING PATIENTS. This form should be completed and handed to the receptionist at your next office visit. [THIS FORM IS NOT FOR PATIENTS WHO ARE NEW TO THE PRACTICE].

MEDICAL RECORDS RELEASE FORM
This form is used when a patient seeks to authorize the release of his/her medical records. Please read and follow the instructions it applies to your individual medical record needs.

WORKERS' COMP FORM
This form is needed to process all Workers' Compensation Claims.

NEW ADULT PATIENT INTAKE FORM
All adult patients WHO ARE NEW TO CNY Family Care should complete this updated patient information form and hand it to the receptionist when they check in for their first appointment.

NOTICE OF PRIVACY PRACTICES
This updated document describes how medical information about patients may be used and disclosed and also how patients can get access to this information.

AUTO CLAIM FORM
As a courtesy to our patients, CNY Family Care will send bills to your auto insurance carrier. This form is required in order for this process to begin. Please complete this form and bring it to the office when you come for your office visit if you would like us to send the bill to your insurance carrier.

AUTH FOR TREATMENT OF MINORS
This form needs to be completed by a parent or guardian of a minor under the age of 18 years of age. It needs to be presented to the front desk staff when a minor comes for a scheduled appointment without his/her parent.
