Licensee (agent/broker/adjuster/consultant etc) - The username is the
numeric portion of your New York Insurance license. The password is the last
4-digits of your SSN and your 8-digit date of birth in (MMDDYYYY)
format. Password Example: SSN 432-98-1234, DOB 08/26/45= 123408261945
Entity/Sublicensee Original - The username is the FEIN of the entity applicant. The password is the last 4 digits of the SSN and the 8-digit date of birth, in MMDDYYYY format, of a sublicensee/designated responsible person applicant.
Entity/Sublicensee Renewal/Relicensing - The username is the numeric portion of the New
York insurance license. The password is the last 4 digits of the SSN and 8-digit date
of birth in MMDDYYYY format, of a sublicensee/designated responsible person.
Insurance Company Appointments/Terminations/Temporary Adjuster Permits - The Department assigned user ID is used for both the username
and password fields. If a user ID is needed for an authorized person of an insurance company to appoint and/or terminate,
please e-mail corporate.affairs@dfs.ny.gov.
Education Providers - The username and password are the user ID created during the registration process. If you have not registered your user information, select Sign Up in the Login box to the right.
Education Providers Original - Select Provider Original Login in the Login box to the right.
