Once you receive your MyChart instructions via email go to mychart.piedmont.org to log on to MyChart. MyChart provides access to your medical health. Please remember, you should not share your password with anyone that you do not want to have access to your private information.
MyChart provides access to your Piedmont medical health record, allows you to schedule appointments, view upcoming appointments, request a prescription refill, view test results, visit summaries and more.
1. Go to mychart.piedmont.org
2. Click on “Sign Up Now”
3. Enter First name, Last name, last four digits of your social security number, and your date of birth.
- You may have issues due to your social. If your social does not work when trying to log-in, please try using “0000” as your social.
- If you still have issues, MyChart questions can be directed to Piedmont Customer Solutions Center at 855-788-1212, menu option 3. Representatives are available Monday – Friday, 7am to 7pm.
If you have an e-mail address on file, then your MyChart Username will be sent to your e-mail account.
