Initial Registration
Initial registrants should come personally to the PRC Central Office or to the nearest Regional Office to file his/her application for registration and to affix his/her signature in the registry book.
All Registration Certificates and PRC ID shall be claimed by the licensee personally at the Central Office, Regional Office or Service Centers where the application for the same has been filed. If there is an unavoidable need for a representative, he must present his/her identification paper with picture and a Special Power of Attorney.

1. INITIAL STEPS IN REGISTRATION
- Search for our website http://online.prc.gov.ph/ and sign up for your account
- For First timer Client: After creating your account Log in and Fill up all the fields required for your transactions and upload your updated photos.
- For Current Client: Enter your existing account and update the information needed and upload your updated photos.
- Select your transaction, appointment date and choose the regional office preferred to come with and select the payment center and save or print the reference number to be used to pay the corresponding amount for your transaction.
- After paying the amount, go back to http://online.prc.gov.ph/ and sign in to print your Oath form.
- Go to your chosen PRC Office and bring your Oath form and Receipt.
2. STEPS IN REGISTRATION
- Complete the INITIAL STEPS IN REGISTRATION.
- Present NOTICE OF ADMISSION / APPLICATION STUB or any GOVERNMENT ISSUED VALID ID together with your OATH Form at Window 7 or 8 (for verification of rating and retrieval of your record card).
- Check your NAME and DATE OF BIRTH from your Oath Form and Registry Sheet, let us know if there is any discrepancy.
- Fill-out the Oath Form properly.
- Secure documentary stamp at the Customer Service Center.
- Submit the properly filled-out OATH FORM at Window 7 or 8 for your registration.
- After registration, receive your claim slip. (check the presence of the following - registration number / date and serial number)

- 3 pieces passport size colored picture in white background with complete name tag (Last Name, First Name, Middle Name) e.g. GAGALAC, ARNEL ROSARIA
- Updated Community Tax Certificate(CEDULA) / One (1) Government Issued Valid IDs (Picture Not Detachable)
- Payment: Registration Fee - Php 600.00 / Annual Fee - Php 450.00
- Documentary Stamp
