We are very excited to announce that the Delta Athletic Department has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.
You will be able to review your information once per school year to verify information is current and sign the forms.
Download the Parent Playbook (attached) to get started in FinalForms. If you require any support during the process, scroll to the bottom of the PDY Final Forms webpage and click “Use Support”.
The only form that is still required to be printed and turned in to the Athletic Department to be filed, is the physical form. The physical form must be completed, signed by student, parent and doctor. Once your student’s physical is received by the athletic department, we will enter it into FinalForms. You will receive automatic notifications when you need to update your student’s forms. All other forms will be completed via Final Forms.
