MyChart keeps your health information in one safe, convenient place, making it easier to stay connected. Here's how MyChart can help you:

Manage your care

Message your providers
Message your provider with your medical questions at any time. Attach a picture or video to help explain your concern, and you'll hear back within three business days.
Read your test results
No more waiting for a phone call or letter. View your results and your doctor’s comments within days.
Review your care plan
When your care team has enrolled you in a care plan to manage your health, you can use MyChart Care Companion to review and follow your plan of care from any smart device.
Visit with your provider online
MyChart lets you be seen by some UI Health Care providers with a telehealth video visit, so you can get the same quality care from the comfort of your home.

Check in early
With MyChart's eCheck-in feature, you can complete your check-in questionnaires early, helping you spend less time at the reception desk.
Manage appointments
You can easily schedule or cancel appointments for many services without having to call. You can also view the details of past and upcoming visits.

Keep track of your family’s medical information directly from your own account. If you are a parent or legal guardian of a child under age 18, you can request access to their medical record by completing and submitting the MyChart Parental/Guardian Access Application form (link above).

Seeing a doctor outside the UI Health Care system? Link your account with other health systems so all your information can be seen by your providers in one place.

How do you sign-up for MyChart

As soon as you schedule your first UI Health Care appointment, you’re eligible for a MyChart account. You can sign up through the MyChart website on your computer, or you can download our mobile app on your iPhone, iPad, or Android device.

No matter which device you use, you can sign up for MyChart using one of two options. Choose the one that fits your situation:

You are a new patient

Before you set up your MyChart account, we'll ask you for personal information to confirm your identity. Once your identity has been confirmed:

  1. On the sign up form, enter all required fields: name, address, date of birth, email address, and phone number.
  2. Tap or click the box next to “I’m not a robot.” The box should change to a green checkmark. You can tap or click the “Next” button. Or you may be asked to pass a simple test in which you identify objects in images. After you’ve done that successfully, the box will change to a green checkmark and you can tap or click the “Next” button.
  3. You’ll be asked a series of questions about yourself. This step helps us make sure that it’s really you creating a MyChart account.
  4. Once you’ve been verified, choose your username and password.
  5. Explore your new MyChart account!

You’ve had an appointment with a UI Health Care provider, and you have your after-visit summary

You should have received an after-visit summary as part of your appointment check-out. If you have an after-visit summary:

  1. On the activation form, enter your activation code, your medical record number, birthdate, and ZIP code.
  2. Tap or click the “Next” button.
  3. Choose your username and password.
  4. Explore your new MyChart account!
